Vaccine database: Arizona’s 2018-19 immunity coverage rates

Each year, schools are required to submit to the state a self-reported survey assessing the coverage and exemption data for child care/preschool, Kindergarten and 6th grade. The data is compiled in the annual Immunization Data Report.

State law allows for religious exemptions for child care, preschool and Head Start programs, medical exemptions that require a documentation, and personal belief exemptions.
